Registration Requirements
2012-2013
3 Year old Pre-School students must be 3 on or before September 1st (Birth date 09/01/2009 or before.)
4 Year old Pre-School students must be 4 on or before September 1st (Birth date 09/01/2008 or before.)
Kindergarten students must be 5 years old on or before September 1st (Birth date 09/01/2007 or before.)
Documents required:
- ORIGINAL/CERTIFIED Birth Certificate (Not hospital Certificate) must be brought into the office WE will make the copy and return it to you (This is a state rule)
- Baptismal Certificate (if baptized-for any religion)
- Social Security Number
- Updated Health Records due no later than the first day of school:
PK3 & 4: Medical, including lead, plus Health History (on side two) filled out & signed by parent
Kindergarten: Dental/Eye/Medical, including lead, plus Health History (on side two) filled out & signed by parent
2nd Grade: Dental
6th Grade: Dental/Medical plus Health History (on side two) filled out & signed by parent
- Transfer Students: A current report card must be presented for the principal's review prior to registration for Kindergarten through 8th grade students. After approval, you may then register and fill out a release of information from which we will send to your former school for transcripts.
A $150.00 PER CHILD New Family/ $100.00 PER CHILD Current Family Non-refundable Registration fee is due at time of registration. The registration fee holds your spot until the registration and fees are completed at Commitment Night May 30 & 31, 2012.
Full Day – PreK 4 through 8th
Hours are 7:50 A.M. to 2:40 P.M.
5 Half Day Pre School - 4 Year old:
Mornings Monday thru Friday
Hours are 7:50 A.M. to 11:00 A.M.
3 Half Day Pre School - 3 Year Old
Mornings on: Monday - Wednesday - Friday
Hours are 7:50 A.M. to 11:00 A.M.
